Minoxidil myths and facts: who it helps, how long it takes, shedding, side effects, oral minoxidil and what happens when you stop.

The short answer

Minoxidil can slow pattern hair loss and regrow some hair, but it doesn’t work for everyone, and it doesn’t treat the cause. It helps male and female pattern hair loss, not shedding from low iron, thyroid problems or stress. Results take four to six months, and the hair gained is lost within months of stopping. Finding out why your hair is thinning comes first.

Does minoxidil work?

For male and female pattern hair loss, yes, for many people. It has more evidence behind it than any other treatment you can buy without a prescription. Some people regrow noticeable hair, many keep what they have, and some see little change. It tends to do more at the crown than at a receding hairline, and more when thinning is caught early. It isn’t a fix for every kind of thinning. Shedding from telogen effluvium, low iron or a thyroid problem needs the cause putting right, and hair lost to scarring alopecia can’t regrow.

Why am I shedding on minoxidil?

Many people shed more hair two to eight weeks after starting. Minoxidil nudges resting hairs into a new growth phase, and the old hairs fall out to make way. It usually settles within a couple of months. Shedding that carries on, or comes with a sore or flaky scalp, is worth having checked.

Is minoxidil safe? The side effects

  • An itchy, red or flaky scalp, the most common side effect. It can also flare eczema, psoriasis or seborrhoeic dermatitis.
  • Unwanted hair on the face or body, more common in women and with oral minoxidil.
  • Dizziness, a fast heartbeat or swollen ankles, rare with the scalp form but more likely with tablets.
  • Not suitable in pregnancy or breastfeeding, and worth checking with your GP if you have a heart condition or low blood pressure.

An inflamed or flaky scalp is more likely to react, which is why I look at the scalp first. Itchy scalp.

What about oral minoxidil?

Low-dose minoxidil tablets are increasingly prescribed for hair loss. In the UK it’s an off-label prescription, so it has to come from a doctor, who should check your health first and monitor you. Unwanted hair growth is common, and it isn’t suitable for people with some heart conditions.

Do you have to use minoxidil forever?

To keep the results, yes. It doesn’t change the cause of pattern hair loss, so when you stop, the follicles gradually return to where they would have been, usually over three to six months. That’s worth knowing before you start.

Does minoxidil work for women?

Yes, for female pattern hair loss it’s one of the best-studied treatments. Women are more likely to notice unwanted facial hair, and it isn’t used in pregnancy or breastfeeding.

How long does minoxidil take to work?

Usually four to six months before you see a difference, and up to a year for the full effect. Early shedding is common.

Can minoxidil make hair loss worse?

It can cause temporary shedding in the first weeks. It doesn’t make pattern hair loss progress faster, but it won’t stop loss with another cause.

Is minoxidil worth trying without a diagnosis?

It’s better to know the cause first. If your thinning is from low iron, a thyroid problem or a scalp condition, minoxidil won’t fix it and could irritate your scalp.
